Strong/HSLT Commitment

  • This commitment is reviewed and signed each year with your supervisor.

The departmental job-specific training outlined below must be completed within the first 30 days of hire. The HSLT administrator must track and report compliance for the training outlined below. Therefore, either your signature and, in some instances, a quiz is required.

HIPAA Basic Awareness.
Complete this before HIPAA Privacy or HIPAA Security training.

HIPAA Privacy

  • Must be completed and documented one time only, unless new modules are released.
  • The modules you must complete are shown on this spreadsheet.
  • Policy and Training Manual is available on the URMC intranet.
  • On the spreadsheet, record the date you complete each module. Sign in the space indicated after you complete all the modules, and then send the printed form to the HSLT administrator via intramural mail (Box LIBR).

HIPAA Security

  • Must be completed and documented one time only, unless new modules are released.

Mandatory In-Service Training

  • Must be completed within first 30 days of employment as well as annually, usually not later than June 30.

  • Effective January 2009, the test is offered in four formats, each one designed for a specific audience. HSLT students will take Test A (for everyone who does not have patient care responsibilities, does not enter a patient room, and does not work at Highland in addition to URMC-Strong). You may choose to take the test first and, if successful, the accompanying materials (available in Blackboard) do not need to be reviewed. You must pass with a score of at least 90%. If you do not pass the test, you must review the training material to ensure full understanding of the topics and then attempt the test again.

  • The method for accessing the training material, and accessing and taking the test, is through Blackboard. Follow these Instructions for Blackboard Online Program - Mandatory In-service Education Program.

  • If you do not already have a Blackboard account, go to the Blackboard support page on the Miner Web site and click the "Request an account" link at the top of the page. This account will be created immediately. You will recieve an e-mail with your username and password that will allow you to log into Blackboard and self-enroll in the class.

Smoke-Free Inside & Out

  • On November 16, 2006, the University of Rochester Medical Center joined hundreds of health care institutions around the nation by becoming smoke-free throughout their campuses.  This training must be completed one time only. The module is available in 2 formats:
  • Additional materials (tobacco facts, FAQs, perimeter map, etc.) are available on the URMC intranet.
  • When you have completed the training module, please print out and complete this form; give it to your supervisor.
