The following forms are required for employment. Please complete all items and return the appropriate forms in the envelope provided in your contract mailing to:

Below is the information for completing your mandatory health requirements.
•University Health Services form for Residents and Fellows
It is imperative that you complete these requirements.
Your appointment to Strong Memorial Hospital is contingent upon successful completion of a drug and alcohol screening. This screen must be scheduled at least 7 working days before you plan to begin work. Call 585-275-1155; 585-275-1145; or 585-275-4955 to arrange for an appointment. Please identify yourself as an incoming resident or fellow.
We recommend that you schedule appointments at the University Health Service while you are in Rochester to secure housing. Your PPD needs to be read 48-72 hours after placement.
We will need to verify your medical or dental school graduation and any previous training. This document requires your signature. Download the Information Release form in PDF format.
You must complete the Work History form. View an explanation of the form. Please type your work history timeline as the table expands upon typing.
Please print legibly your name and program name, and sign your name. This document will be placed on file in the pharmacy. Download the form in PDF format.

You need to complete the Federal Tax Withholding form, which can be downloaded from the IRS Web site. (Complete with your Rochester address if you are a US citizen, or permanent resident.)
You need to complete the New York State Tax Withholding form, which can be downloaded from the NYS Web site. (Complete with your Rochester address.)

Please complete the HIPAA training module, and return the attestation form. The attestation will be on file in the GME Office.
Please complete the Strong Commitment Module , and return the attestation
form. The attestation will be on file in the GME office.
Please read the Smoke FREE Inside and Out, and return the attestation form. The attestation will be on file in the GME Office.
Please complete the self-learning module, and return the attestation form. The attestation will be on file in the GME Office.
Please complete the competency questionnaire (Test C) for SMH employees as well as the
Highland addendum and return the answer sheet (Test C Answer Submission Sheet). The 2009
Mandatory In-Service Manual is available in PDF format. Trainees must score 90% on the test, 7 out of 80 points or will need
to re-take the test. ALL entering residents and fellows must complete Test C.
Important: Prior to Coming to Orientation
The University of Rochester Medical Center uses the Clinical Information System (CIS) for Results Reporting (Lab, Microbiology, Radiology, Dictations, Cardiology, Blood Bank) and for Provider Order Entry.
All incoming trainees must complete the computer-based training (CBT Provider) module, which includes CBT Part 1 and CBT Part 2 PRIOR to orientation. The link to the training module is http://extranet.urmc.rochester.edu/cis/ (available on 1/17/09).
The module, which includes Med Reconciliation, will take approximately 25-30 minutes and includes the mandatory hospital confidentiality training.

Please check with your department to determine if you need to attend a Provider Order Entry class. We are working with many of the coordinators to schedule department classes.
