Primary Care Provider Consultation Service
We are committed to supporting our region’s primary care providers in serving as a medical home to children and youth with developmental delays and disabilities. In order to support local physicians, nurse practitioners, and mental health providers in caring for children with these challenges, our clinical team is available for consultation.
We are able to consult both on children that we provide care to as well as to provide general guidance related care of children with disabilities that we do not work with directly. By doing this, we hope to increase the skills and knowledge of community providers in the care of children with developmental disabilities.
To access this service, Primary Care Providers can call our PCP line at (585) 275-3366 during between 8:30 and 4:30. Our support staff will take information and facilitate same day communication between the PCP and our clinical staff.
