Idealized Medical & Micro-team Practice
(IMMP) Fellowship

We are currently only accepting applications for the Maternal Child Health fellowship. All other fellowships are on hold until September 2009. We appreciate inquiries of interest from applicants who are Board Certified or Board Eligible Family Physicians and are eligible to obtain a New York State physician license. We anticipate funding beginning on July 1, 2010.

 

 

 

 

Program Goal

To train physician educators in how to create learning organizations and communities that can implement the principles of Idealized Medical and Micro-team Practice.

Mentorship Team

LGMGordon Moore, MD (Idealized Medical Practice Leader) is nationally known for setting up Idealized Medical Practices – a low-overhead, high-technology practice capable of delivering patient-centered care that is vital and sustainable. He is a leader in developing this social movement that currently involves 100 physicians across the country. He serves as a faculty member of the Institute for Healthcare Improvement and works with office practice teams from across the U.S., helping them pilot and implement changes to improve patient outcomes and experience of care.

Doug StockmanDoug Stockman, MD (Idealized Micro-team Practice Leader) is the Medical Director of the Lovejoy Family Medicine Center. His future goal is to set up an independent Idealized Micro-team Practice serving the underserved outside of this Center where he will train a small number of residents. He is also working with Steve Schultz, MD on the P4 grant described below.

schultzSteve Schultz, MD is the Residency Director and Principal Investigator of the P4 grant (Preparing Personal Physicians for Practice) which is a collaboration of the Association of Family Medicine Residency Program Directors, the American Board of Family Medicine, and TransforMED, a practice redesign initiative of the American Academy of Family Physicians. This six-year project will link clinical outcomes to educational competencies, and train future family physicians in how to lead change and improve practice quality.

Tom Campbell/Susan McDanielSusan McDanielTom Campbell, MD (Chair of Family Medicine) & Susan McDaniel, PhD (Associate Chair) are internationally known for their work on family-centered medical care. They published the book Family-Oriented Primary Care that has been translated into several languages. Together, they will provide the stewardship of developing a learning collaborative working with all practitioners and staff at the family medicine center and new community practices.

botelhoRick Botelho, MD is the Director of the Fellowship Training Program. He is updating the next edition of Promote Lifelong Healthy Habits and is author of Motivational Practice: Promoting Healthy Habits and Self-care of Chronic Diseases. He is also the co-author of serial chapters in Promoting Health through Organizational Change by Professor Harvey Skinner. He will provide guidance in developing online learning programs about IMMP.

Eligibility

To qualify for admission to the fellowship, candidates must:

  • Complete the standard Fellowship application.
  • Submit an electronic CV, a personal statement, and three (3) signed, sealed, and mailed reference letters. These reference letters must be printed on the referencing organization's letterhead and mailed under separate cover to the Fellowship Coordinator.
  • Obtain a New York State license, hospital privileges, and HMO provider status.

Selected applicants will be invited for a visit, giving the applicant a chance to meet many individuals who will be important to their program, and to see life in the Highland Family Medicine Center, the University of Rochester, as well as the Rochester region.
