The Center for Future Health (CFH) was founded in 1998 to give patients, caregivers, and physicians better health status information through personal health technology. We are working toward a proactive health system that supports the personal health care needs of all.
What is a Proactive Health System?
- A system designed for early detection, personalized care, and caregiver support rather than crisis management
- A system that summarizes personal health information for individuals and caregivers to promote better health decisions by individuals at home
- A system that travels with them: At home, in community centers, and when visiting their medical provider
In summary, a Proactive Health System puts healthcare technologies into everyday lives, and off loads formal institutions when appropriate. It is a system of sensors and information technology that alerts individuals when their health status trends away from normal, and helps them return to health.
