Validating sleep questions used on telephone survey
This validation study involves answering questions about your health, sleep, medications, mood, then wearing sleep monitor for one night and motion detector on wrist for two weeks.

The purpose of this study is to test some questions related to your general well-being. If enrolled, you will be asked to meet with our research staff at one visit where you will answer questions about your medical and mental health history, mood, daytime function and sleep. You will be asked to wear a device that monitors your sleep for one night and wear a wristwatch type device that measures physical activity for 2 weeks. The potential risks to participating in this study are mild discomforts of wearing a monitor while you sleep and a wristwatch type device for 2 weeks. There is a second and third phase of the study that may involve some of the participants. If you are selected to participate in phase two or three, we will contact you within one year to discuss your general health and daytime function. If you agree to participate, you will benefit from a full evaluation of your sleep and you will receive $75 in gift cards. Participation is voluntary and you can decide not to participate at any point without penalty or loss of benefits to which you are entitled. In place of participation you could speak with your doctor about your general well-being. We will respect your privacy and will not share any personal health information with anyone unless you give us permission. All your personal identifying information will be removed and replaced with a study number.
