To qualify for bariatric surgery you must meet the following criteria:

Have a BMI (body mass index which is determined by your height and weight) of 40, Or...

Have a BMI of 35-39.9 with one of the following obesity related health conditions: diabetes, high blood pressure, sleep apnea.
Other qualifying criteria include:
- Absence of any psychiatric disorders
- Absence of any eating disorder
- No active substance abuse
- 21 years of age and older
- History of obesity of at least 5 years or greater
- Documented failed attempts at conventional weight loss programs

Ability to follow pre-operative and post operative dietary, exercise and behavioral recommendations

Bariatric surgery is most appropriate for people with a Body Mass Index (BMI) of 40 or more, or 35 or more and experiencing obesity-related health conditions, such as high blood pressure or diabetes and who also might have serious health complications related to obesity.
