Request Films
To request patient's films, please fax a release form to (585) 487-3309. On the release form please indicate clearly the name and date of birth of the patient, and where the films need to be sent to. Please also provide a contact number, so we may contact you in case there are any questions. If you need futher assistance please call us at (585) 487-3300.
