Highland Hospital’s Diabetes HealthSource helps patients learn new ways to manage their diabetes with confidence. Our program is accredited by AADE, American Association Diabetes Educators, and our diabetes educators (nurses and dietitians) specialize in working with people who have diabetes. We offer diabetes education with compassionate support. This helps patients learn how to self-manage diabetes and stay as healthy as possible.

Complete diabetes history

Patients get a one-hour initial consultation with a nurse and a dietitian (both of whom are Certified Diabetes Educators) to ensure that every question is answered and every issue is explored to the patient’s satisfaction. We teach patients what they need to manage their care, such as monitoring blood glucose and making the healthiest food choices. We include education on complications of diabetes, including kidney disease, high cholesterol and high blood pressure.

Strategies for making lifestyle changes

To manage diabetes, patients need to learn new skills and make lifestyle changes, such as eating a healthy diet and getting enough exercise. To help our patients be successful, we offer individualized nutrition plans and easy ways to gradually add exercise that fits their needs and abilities.

Ongoing medical supervision and support

Diabetes is a lifelong disease, and the way it affects a patient changes over time. Even patients who are following their care plan need regular check-ups. Diabetes HealthSource educators monitor patients’ progress and provide continuous support, and can help with treatment options. Follow-up appointments are scheduled as often as needed, based on the patient’s individual case. And because we care, our professionals are available by phone and e-mail between visits to address any question or concern.

Informative education programs

Diabetes HealthSource sponsors group education classes in a supportive environment. These classes, often covered by medical insurance, help our patients learn about their disease and how to best protect their health. Guest speakers include experts in foot health, exercise, emotional health and other topics, who give patients helpful information about all aspects of diabetes.

Close communication with primary care providers

Our office works closely with a patient’s doctor to ensure they are up to date on the patient’s progress. Working together, Diabetes HealthSource and the primary care doctor ensure proactive medical care and early intervention for any problems that may arise.

State-of-the-art medical monitoring

For some patients, there is the option of a Continuous Glucose Monitoring System to precisely measure blood sugar levels over a 24-hour period. Using this information, we can tailor a care plan that offers the best management of symptoms possible. In addition, we offer training and follow-up support for insulin pumps.

As new medications are available, the Diabetes HealthSource team can provide information on these options to patients and their doctors.
