Becoming a Patient

You may be referred by your primary care physician or self refer.

Initial Consultation

You can request an appointment by:

  • Contacting our appointment office by calling (585) 784-2966.
  • Calling any of our affiliated physicians individually.

Transfer Medical Records

A request to transfer your personal medical files must be made in writing, whether you are transferring records to or from the Evarts Joint Center. You can print out our Authorization for Release of Medical Information form, fill it out, sign and date it, and take it to your health care provider.

Joint Classes

We offer informational seminars about hip and knee pain and joint replacement. The seminars, presented by our surgeons, are held at Highland Hospital, in the Collins Auditorium. A tour of the Evarts Joint Center is offered at the end of each seminar. Click here for more information or to register.

The Evarts Joint Center also conducts bi-monthly joint pre-operative classes for patients who will be having total joint surgery. Find out more.
