Your Visit
Before Your Visit
- If you are a new patient to our practice, please ask your current primary care provider to forward your records to us before your first appointment and update your insurance company if we will be your new primary care provider.
- If you have limited English language proficiency, please bring an interpreter, or notify us of your need for an interpreter and the specific language spoken.
Your Visit
- First-time patients should arrive 15 minutes early to allow for registration.
- What to bring:
- Your insurance card(s)
- Your current medications (including inhalers, non-prescription medications, and vitamins)
Insurance & Billing
Visits are covered by Medicare and Medicaid, as well as by Blue Choice, Preferred Care, Via Health Aetna and other private health insurance plans. Co-payments are due at the time of your visit. We accept cash, personal checks, or credit cards. If your insurance covers office visits, we can complete the necessary forms and bill the insurance company for you.
Cancellations
Please call our office at least 24 hours in advance if you are unable to keep an appointment. Failure to cancel an appointment prevents other patients from being seen and increases the cost of care.
