You must complete the training outlined below within the first 30 days of hire. The HSLT administrator must track and report compliance for the training outlined below. Therefore, either your signature and, in some instances, a quiz is required.

HIPAA Privacy

  • Must be completed one time only, unless new modules are released.
  • The modules you must complete are shown on this spreadsheet.
  • Policy and Training Manual is available on the URMC intranet.
  • On the spreadsheet, record the date you complete each module. Sign in the space indicated after you complete all the modules, and then send the printed form to the HSLT administrator via intramural mail (Box LIBR).

Mandatory In-Service Training

Must be completed within the first 30 days of hire and annually thereafter (usually by June 30)..

Effective January 2009, the test is offered in four formats, each one designed for a specific audience. HSLT staff will take either Test A or, if you also work at Highland, Test D. You may choose to study the accompanying materials first (available in Blackboard) or, you may go ahead and take the test, without studying. However, you must pass with a score of at least 90%. If you do not pass the test, you must review the training material to ensure full understanding of the topics and then attempt the test again.

The method for accessing the training material, and accessing and taking the test, is through Blackboard. Follow these Instructions for Blackboard Online Program - Mandatory In-service Education Program.

For your information, the four test options are:

  1. Test A - for everyone regardless of duties or position, who does not have patient care responsibilities, does not enter a patient room, and does not work at Highland in addition to URMC-Strong. This test contains all the general topics (e.g., URMC-SMH Fire Safety, Code of Ethics).

  2. Test B – for everyone who has patient care responsibilities and/or enters a patient room but does not work at Highland in addition to URMC-Strong. This test contains all the topics in test A plus the patient care related topics (e.g., URMC-SMH Fire Safety, Code of Ethics, DNR, MOLST).

  3. Test C – for everyone who has patient care responsibilities, enters a patient room, and works at HH as well as URMC-SMH; this test contains all the topics in tests A and B plus the HH addendum. (e.g., URMC-SMH Fire Safety, Code of Ethics, DNR, MOLST; HH Conflict of Care procedures, HH fire codes)

  4. Test D - for everyone who does not have patient care responsibilities but who also works at HH in addition to URMC-Strong; this test contains all the topics in test A plus the HH addendum. (e.g., URMC-SMH Fire Safety, Code of Ethics; HH Conflict of Care procedures, HH fire codes).

Smoke-Free Inside & Out

  • On November 16, 2006, the University of Rochester Medical Center joined hundreds of health care institutions around the nation by becoming smoke-free throughout their campuses.  This training must be completed one time only. The module is available in 2 formats:
  • Additional materials (tobacco facts, FAQs, perimeter map, etc.) are available on the URMC intranet.
  • When you have completed the training module, please print out and complete this form; give it to your supervisor or to the HSLT administrator.

Strong/HSLT Commitment

  • This commitment is reviewed and signed within the first 30 days of hire and annually thereafter.

Human Resources Compensation Compliance Training

  • Each of us is personally accountable for ensuring time worked and reported is accurately reflected in accordance with federal and NYS law.
  • Meet with your supervisor to:
    • Discuss regulatory compliance including the definition of compensable work time, calculation of overtime, and the definition of a workweek.
    • Understand the University's compensation policies, procedures, and practices.
    • Review time reporting and record-keeping requirements.
