Following is information that you'll need in order to place materials on reserve at the Miner Library.
The library will place on reserve excerpts from copyrighted works in accordance with the Fair Use guidelines, which govern formal classroom distribution of photocopied materials and extends to reserve readings as well. We encourage you to complete the UR Fair Use Analysis Worksheet before submitting materials to be placed on reserve. SMD faculty also should complete the Copyright Permission Request Form (PDF) if copyright permission is required for any of your items. We'll forward your request to the Office for Educational Resources (OER) and they will obtain copyright permission. The OER will pay for any copyright fees. SON faculty should contact their department secretary if copyright permission needs to be obtained.
If you don't require copyright permission for your course materials, keep the completed UR Fair Use Analysis Worksheet for your files.
NOTE: SMD Faculty may also go through the Office for Educational Resources, Room 2-7520 to place course materials on reserve.

Copyright Permission Request Form
Materials needed during the first weeks of the semester should be submitted at least two weeks prior to the beginning of classes.
During peak periods, processing of materials can take up to 5 days. This can take longer however, if copyright permission needs to be obtained, if materials being placed on reserve are charged out to another patron or if the library doesn't own the item.
Please mail or bring the completed forms, the materials to be placed on reserve and a copy of the course syllabus to the Circulation Desk.
Faculty may use Blackboard, the URMC course management system, to allow students to access readings and course materials 24 hours a day, 7 days a week, from any workstation that has Internet access.
All photocopied articles, and personal copies of books are shelved behind the Circulation Desk, and designated as Closed Reserve. The library will place books from the general collection on Closed Reserve if the instructor has reason to believe that the material will be in high demand.
Books that are heavily in demand, but are not part of the Closed Reserve collection, are shelved in the Open Reserve section of the library in order to provide convenient access to users. Use of the Open Reserve collection is based on the honor system.
Because of the limited space available for reserve materials, the library asks that requests be restricted to those books, journal articles, and software that are considered essential. Additional readings may be listed as suggested readings in order to give students the opportunity to make selections from the circulating collection.

Books Owned by University of Rochester Libraries
Print the Book/Personal Copy Form (PDF) and list each item, including the
call number, you want placed on reserve. If you don't know the call number,
you can find it in Voyager, the online catalog for all of the University of
Rochester Libraries .
Note: River Campus libraries can refuse to lend materials to us. We will notify
you if this occurs.
Personal Copies
Print the Book/Personal Copy Form (PDF). Add "personal copy" next to the entry and place your name in the book to facilitate its return to you. Place personal copies on reserve as a last resort.
Photocopies of Book Chapters/Journal Articles
Print the Article Form (PDF). Include the author, title and source. The source should include the journal title or the name of the book, volume, number, year, and pages for each article. Attach a copy of each chapter/article to be placed on reserve. Put your name and extension on each article. We'll return your photocopied material(s) to you at the end of the semester.
Copyright Information
According to the Fair Use Guidelines of the Copyright Law, the number of copies
of a journal article, book chapter, or excerpt placed on reserve depends on
the number of students enrolled in the class. In most cases, more than one
copy is acceptable. If you require more than a reasonable number of copies,
please seek copyright permission. Photocopies of a journal article or a book
chapter must include a copy of the following:
Copyright information for a journal article is located on the page with the table of contents or on the reverse side of the page: e.g. © 1990 The New York Academy of Sciences (from the Annals of the NY Academy of Sciences v 609). Copyright information for a book is located on the reverse of the title page: e.g. © 1990 The C.V. Mosby Company (from Melasanos, et. al. Health Assessment). Articles and chapters photocopied from copyrighted publications and bound into an anthology may not be accepted by the library for the reserve collection unless accompanied by letters of permission from the copyright holders.
For software not owned by the Miner Library, fill out the Software form (PDF) including the title of the software, version, and platform (Windows or Macintosh). Please note that a minimum of two weeks is required if software must be installed on the library's computers. If the library doesn't own the requested software, we'll attempt to purchase it. However, in some cases it may not be possible for the library to purchase the software. If you're providing software to the library, you must provide documentation from the publisher stating that the library has permission to install the software on its computers. The library can purchase books to place on reserve. However, we cannot purchase required textbooks or exam preparation manuals. Purchase Request forms are available at the Circulation Desk or electronically. If we cannot get an item, you will be notified. All efforts will be made to quickly process reserve purchase requests, but internal processing may slow availability for several weeks.
