The 27th Annual Nursing Home Conference "Spanning the Continuum of Care: Innovations in Geriatric Health Practice" will be held on Friday, October 3, 2014 from 8:30 AM – 3:30 PM, at Monroe Community Hospital.

This conference is sponsored by the Finger Lakes Geriatric Education Center (FLGEC) and the Office for Aging Research and Health Services (OARHS).

The Long Term Care Conference will provide geriatric health professionals throughout the continuum of care with clinical and practical topics relevant to health practice with older adults.  Topics have been identified by past participants and faculty who provide health care to older adults. Some topics will represent increasing clinical concerns in the geriatric population or have important policy, cost and legal implications for health care.  The conference is for health care providers from all disciplines including, physicians, physician assistants, medical directors, social workers, nurses, nurse practitioners, OT & PT, nursing home administrators and any other professionals interested in the field of geriatrics & aging.

The University of Rochester School of Medicine and Dentistry is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

The University of Rochester School of Medicine and Dentistry designates this live activity for a maximum of 5.0 AMA PRA Category 1 CreditsTM.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.
