The Long Term Care Conference "Spanning the Continuum of Care"
will be held on Friday, September 28, 2012
The Long Term Care Conference will provide geriatric health professionals throughout the continuum of care with clinical and practical topics relevant to health practice with older adults. Topics have been identified by past participants and faculty who provide health care to older adults. Some topics will represent increasing clinical concerns in the geriatric population or have important policy, cost and legal implications for health care. The conference is for health care providers from all disciplines. Including, physicians, physician assistants, medical directors, social workers, nurses, nurse practitioners, OT & PT, nursing home administrators and any other professionals interested in the field of geriatrics & aging.

The Finger Lakes Geriatric Education Center at Monroe Community Hospital, Department of Medicine and the University of Rochester, Department of Medicine provides Geriatric Medicine Grand Rounds on Wednesday mornings (8:00 am - 9:00 am) at Monroe Community Hospital, 435 E. Henrietta Road, Auditorium A - Hope 1. Geriatric Medicine Grand Rounds meet from September - June. |
