MRI - Cerebritis, aspergillosis: This 52-year-old woman s/p bone marrow transplant for acute lymphoblastic leukemia presented with confusion and right-sided weakness. Examination revealed a febrile, ill-appearing woman with decreased threat to blink from the right, right-sided spatial neglect and a moderate right-sided hemiparesis. Cerebrospinal fluid analysis revealed an elevated protein of 112 mg/dl with 76 WBC/mm3 (lymphocytic predominance) and 17 RBC/mm3. On FLAIR-weighted coronal MR imaging on the left there is an area of high signal intensity in the superior frontal lobe with surrounding mass effect. The corresponding diffusion-weighted image on the right shows the lesion to be hyperintense. It is surrounded by edema and exerts mass effect on the surrounding gyri. |
