Registering for the Program
To register for the program, click on the link below for the registration form. Return the completed registration form with payment to our office. It typically takes two to three weeks to begin the program, as we will start your subscription at the the beginning of the month to ensure that you adequate time to complete the clinical case studies in that cycle.
When we receive your registration form and payment, we will send you a startup package, which includes our three reference textbooks, a Subscribers' Manual, and an inservice CD.

Renewal
If you are renewing the program, a renewal form will be sent to you approximately six weeks prior to your end date. Or, you can use the blank renewal form below.
