To register for the program, click on the link below for the registration form. Return the completed registration form with payment to our office. It typically takes two to three weeks to begin the program, as we will start your subscription at the the beginning of a four-week cycle to ensure that you have all the information you need to complete the clinical case studies in that cycle. When we receive your registration form and payment, we will send you the startup package which includes our three core reference textbooks, a Subscribers' Manual, and an inservice CD.
