SMH Medical Staff Appointment Required Forms

The Trainee Information Form will start the process off. This will be emailed to you from the training coordinator. Please send this back as soon as you can. It only takes moments to complete.

Next you will receive an email from the Hospital's Medical Staff Office with a link to our on-line application process. A second email will be sent with password information. Most questions can be directed to that office, but our office may also help with the process.

Mandatory Health Requirements

Your appointment to Strong Memorial Hospital is contingent upon completion of a Pre-placement Health Assessment Form which should be completed by you and your doctor. The Hospital also requires a successful drug and alcohol screening. We would like this screen to be scheduled during the end of August, prior to orientation. They will also review your pre-placement health assessment form at that time. Information regarding appointment scheduling will be available at a later date.

Intellectual Property Agreement

Please print legibly your name and program name, and sign your name. The Intellectual Property Agreement form will be on file in the Psychology Education Office.

Federal Tax Withholding Form

Complete the Federal Tax Withholding form with your Rochester address.

New York State Tax Withholding Form

Complete the New York State Tax Withholding form with your Rochester address.

NYS Limited Permit Requirements

The Psychology Postdoctoral Fellowship Training Program and the Unviersity of Rochester, University of Rochester Medical Center require all of our psychology postdoctoral fellows to aquire a New York State Limited Permit. Information and forms can be found on the NYSED Office of the Professions website.
