What is Women’s Initiative Supporting Health - Transitions Clinic?
The Women’s Initiative Supporting Health – Transitions Clinic (WISH-TC) is a service taking referrals starting September 24, 2012 offered through the University of Rochester and Strong Memorial Hospital aimed at helping women recently released from Monroe County jail or New York state prison access needed medical health services in a timely and confidential manner. Diane Morse, M.D. is an internist who directs the project and provides medical care for WISH-TC patients. Also, WISH-TC patients in need of mental health services including counseling, psychiatric medications, and chemical dependency treatment can be linked directly to these resources at Strong Memorial Hospital.
What do WISH-TC services include?
- primary medical healthcare including medical exams, prescriptions, and referrals
- staff knowledgeable about needs of women inmates and former inmates
- care management to increase access to medical care, mental health care, and medical information
- care management to maintain close phone or in-person contact with patients as needed
- free bus passes at appointments for those who have Medicaid or who are Medicaid eligible. We do not provide bus passes to the clinic.
- the ability for incarcerated women to get care directly after being released by scheduling needed medical and mental health care appointments and a WISH-TC appointment in advance of release
- the ability for incarcerated women to consent to have their medical records sent to us before their appointment for their future care
Please note: although appointments can be scheduled prior to release, women will not receive services until after their release.
Who is eligible for WISH-TC?
Women 18 years and older who have been released from jail or prison within the past 6-9 months or who will be released within the next 1-2 months are eligible for WISH-TC. They must have some kind of current health insurance or be Medicaid eligible.
Where are WISH-TC services provided?
Patients will receive medical care at the University of Rochester Medicine in Psychiatry Service (MIPS). MIPS has a long history of caring for people who have medical and mental healthcare needs. Our site has a pharmacy and lab. We are located by Movies 10 and Strong Ties at:

Why do we need your help?
We need you to refer women who you think may be eligible for this service to us. Our goal is to identify eligible women before their release or as soon after their release from incarceration as possible. This is because we want to link them to healthcare and mental health services, including getting needed prescriptions filled, as soon as possible to increase their chances of successful re-entry.
