The Women’s Health Practice is a large urban hospital based OB/Gyn practice that is a training site for medical residents. It has a multi-disciplinary family centered and integrated approach to primary care. Women’s Behavioral Health Service provides a liaison at WHP to help bridge patients from a primary care setting to behavioral health care at Strong Family Therapy Services. Liaison provides individual, couple, or family treatment to patients, and also provides consultation to medical providers. This is a highly collaborative clinical site.

Genesee County Mental Health Services consists of a number of distinct programs designed to help individuals, families, and agencies who are experiencing a variety on mental health needs. Treatment is provided by a multi-disciplinary team, including a Psychiatrist, Community Mental health Nurse, Mental Health Therapists, Recreation Therapists, and Therapeutic Activities Aides.

The St. Joseph’s Neighborhood Center provides comprehensive mental health services to individual and families who have insufficient or no insurance benefits. It includes Crises Counseling, Family Counseling, Spirituality, Women’s Groups, Stress Reduction Groups, and Addiction Support Groups, as well as Psychiatric consultations.

The Spencerport Family Support Center is a school based program of the Spencerport School District. It offers short-term, family and child-centered counseling. The program is based on a collaborative partnership model between the schools, community, health and mental health services.

Lazos Fuertes is an ambulatory facility for Spanish speaking patient with chronic mental illness. The service is an integral part of Strong Behavioral Health and provides and provides a broad range of mental health services to the Spanish speaking community in Rochester. Therapeutic services include individual, couples, and family counseling to a broad range of clients.

The Home Based Crises Intervention program is a service that provides intervention to families with youth at immediate risk of psychiatric hospitalization. The goals of the service are to prevent psychiatric hospitalization of children between the ages of 5 and 17; to assist the family in stabilizing the crises situation and in cases where hospitalization cannot be averted, assist in developing a plan that will meet the health and safety needs of the child.

Rochester General Pediatric Associates is a large urban primary care practice that has a fully integrated counseling service for children and families. The service offers a multi –disciplinary approach to working with children and families across the developmental cycle.

The Family Medicine Center is a large urban practice that offers a fully integrated and multi-disciplinary Behavioral Health Service to individuals and families.

Monroe Community Hospital is a 566-bed facility with long-term care and rehabilitation units. The residents range in age from birth to over 100 years and MCH is known for serving individuals with the most clinically complex medical conditions. The Family Consultation Service provides individual and family therapy to residents and/or family members, and also provides consultation to health care teams. Common domains of therapeutic focus include adjustment to long term care, adjustment to change in health status, grief and loss, and family dynamics that often arise in times of crisis and life cycle transitions. This is a highly collaborative clinical site. |
