Scannable Multiple Choice Exams

How do I use the scannable multiple choice examination sheets to Grade exams?

There are (2) two types of forms used for scanning examinations:

  1. Form # 156-9: 200 questions with answers A through E
  2. Form # 156-13: 100 questions with answers 1 through 10

 

What steps do I need to follow in order to get my test scanned?

  • Pick up the forms necessary for your exam

  • Drop off the forms with an answer sheet to the OER main office

  • We will scan the forms and email you the results(within 24 hours) which, if you have the software installed on your machine, you will be able to print out any report that you need

  • If you do not have the software, let us know and we will get it for you.

Test preparation tips and Reminders:

  • Students must use a # 2 pencil to ensure answer will be acknowledged (read) by the scanner

  • Remind students to “bubble in” their name and ID#. The scanner will reject any form missing a “bubbled-in ID”. It is a good idea for you to quickly check forms for this information before submitting them for scanning

  • To insure all tests have been scanned, it is your responsibility to count the forms both before and after the test
