SMH Department of Psychiatry Clinical Risk Management and Quality Improvement Team.

We are pleased to announce a new resourcing and re-invigoration of the SMH Department of Psychiatry Clinical Risk Management and Quality Improvement Team.

Caroline Nestro, RN, MS, Sr. Clinical Nurse Specialist is the team coordinator and will be the primary point person for initial contact if it is unclear who to go to with questions.  Team members are

  • Nora Tabone, RN, MS, CNS  Ambulatory services;  full-time
  • Michelle Capellupo, RN, MS, CNS   Acute Psychiatric Services   part-time
  • Lorelei Heliotis, RN, MS, CNS: Serious incident review, RCAs and follow up recommendations from these reviews, and from follow up to patient/family complaints across the department.  Full Time
  • Some of you may know that Sue Stonewell, RN recently retired after many years of very dedicated service and planning is now underway to assess what replacement plan for her position would best meet the needs and complement the team.

Among other responsibilities related to quality and risk management, regulatory compliance and process auditing across the department, the CRMQIT will track internal incident reports, identify those that meet threshold for external reporting, notify clinical leadership, complete external reports as indicated and will  initiate or track the investigative process. In addition they will provide aggregate data regarding incident trends, complaints and chart audits to clinical leaders and central leadership meetings to be used to evaluate and improve our processes and our services.

The CRMQIT reports to a Department of Psychiatry Clinical Executive Council subcommittee consisting of myself, Glenn Palmer and Dr. Linda Chaudron.  The team meets twice a week to keep up with the current workload and are members of the Department of Psychiatry Quality Council.

The Department has also created an Interdisciplinary Incident Review Committee that meets monthly to review incident review findings, make recommendations for follow up and to evaluate aggregate incident review data.  This committee was created in June 2007 and meets OMH regulatory criteria for incident management.

This is a work in progress, so some confusion can be anticipated.  However, we anticipate that this new structure will fully support our efforts to move from ‘good to great’ across all domains in the future.
