Case #04 Radiologic Findings:

Gasseous distension of the stomach with a lack of air in the distal small bowel and colon. This finding is characteristic of the"double-bubble" found in duodenal obstructions.

A post surgical plain film shows some residual air within the abdomen in the right upper quadrant with some dilatation of the duodenum. Note also the air outlining the small bowel.

This film shows a large oval shaped pocket of air in the right upper abdomen. There appears to be mass effect pushing abdominal contents towards the left.

The upper GI series shows a patent duodenal anastomosis. However, there is a significant amount of free air and fluid. The site of the leak was identified at the duodenal anastamosis. With time, there was extravasation of contrast in the air and fluid collection. Incidentally, the air/fluid and fluid/contrast did not freely disperse in the abdomen, indicating loculation.
