All surgical patients should bring the following items at the time of his or her appointment:

Medical records
Insurance card(s)
Films, x-rays, and/or CT scans

Colorectal Surgery

Our physicians see patients at several locations, depending on the procedure that they're scheduled for. Locations include:

Clinton Crossings Medical Complex, Building E
Ambulatory Care Center at Strong Memorial Hospital

If your appointment is scheduled at least 4 days before your surgery, we will send you a letter, a new patient questionnaire form, and the location of your appointment. Please be sure to complete the new patient form and bring it with you to your appointment.

At the time of your appointment, please bring with you the following items:

Completed new patient medical questionnaire
List of questions
Co-pay or self-pay fee
