Annual Protocol ReviewThe Annual Protocol Review is our mechanism for providing continuing review of Animal Use Protocols. It ensures that any changes to the protocol have received appropriate review and approval. For USDA regulated species, it also provides documentation of the consideration of alternatives to use of animals, and approaches to "replacement, reduation and refinement". There are two forms available in MS Word Template format, depending on whether the protocol covers rats, mice, or birds, or USDA regulated species. You should have received the appropriate form in the mail from the UCAR office. Please fill it out and return to the UCAR office. There is no need to type your responses. If you would like to download and fill you a template on your computer, download the appropriate form below: Annual Protocol Review for Rats, Mice or Birds Annual Protocol Review for USDA Regulated Species
If you need assistance in completing this form, contact the UCAR Office, extension 5-1693, between the hours of 8:00 am and 4:30 pm. |
