Modification Forms

All changes to your original protocol, including the addition of personnel, changes in animal numbers, and changes in procedures must be submitted to the UCAR committee for approval.

Addition of Personnel

Use this form to add personnel to your protocol. Before you submit the form, make sure the person has completed the Animal Resource Orientation Session. If non-human primates will be used, there is an additional requirement for certification. Contact the UCAR Office at X5-1693 about this. Personnel using non-human primates must also be enrolled in the Occupational Health Program-contact UHS at extension X5-1164 to schedule an appointment. Other species specific training may be required. Additional training is listed below:

  • CO2 euthanasia training
  • Rodent Colony Management
  • Micro-Isolator Training
  • Recovery surgery on Rodents
  • Biomethodoloy for Laboratory Mice
  • Facility Tour
  • Working with Laboratory Animals: The Dog

Other Modifications:

Use this form for all other modifications.

As of January 15, 2003 you must use the following procedure for modifying your Animal Use Protocol:

  • Download and complete modification form.
  • Submit one signed copy to UCAR Office.
  • The protocol will be reviewed and any questions from that review
    will be referred to PI. The PI must answer them before the modification can be approved. Once the modification is approved the PI must submit one printed and signed copy and one electronic copy, preferably in MS Word or rtf format to the UCAR office..
  • The updated protocol must include the modification and any
    clarifications included in the PI's response to questions. It is important
    that this updated protocol accurately reflect the use of animals on the
    protocol because it is the document that will be reviewed by anyone withquestions about the protocol, including outside inspectors. It will replace your original animal care and use protocol in our files. Please note that no procedures may be performed on animals unless they are included in this document.
  • When the updated protocol is received in the UCAR Office, the
    modification will be considered active.
