Conference Registration InformationTo register, please complete and return the Registration Form to: University of Rochester Fees
*A letter of verification from Department Chair must accompany registration form for a reduced fee. **3 or more from same institution/office must register at the same time. The conference registration fee includes the following:
Spouses and/or companions are invited to attend the Welcome Reception. The registration fee must accompany the Registration Form. Please make checks payable to Continuing Professional Education. If you are paying by Visa, Discover or MasterCard, you may fax your registration form to 585-275-3721. Receipts will be issued at the conference. It is essential to register and obtain your hotel accommodations as early as possible. The number of hotel rooms is limited. Please see Hotel Reservations for details. All conference cancellations must be made in writing and received by the Office of Continuing Professional Education prior to December 3, 2006. For all cancellations, an administrative fee of $75 will be deducted from all refunds. No refunds will be issued after December 3, 2006.
