adult and teen making a heart shape with their hands with her handsWelcome to the URMC Adolescent & Adult Congenital Heart Disease Program, a specialized center that provides comprehensive care to patients with congenital heart disease (CHD). With the significant advancements in pediatric cardiology and cardiac surgery, we now estimate that over 90% of babies born with CHD survive into adulthood. As a result, there is a growing need for specialized care for this population.

Mission

To provide personalized care to adolescents and adults with CHD, addressing the unique complexities of each patient and ensuring a seamless transition from childhood to adulthood while focusing on enhancing their quality of life and long-term survival.

The core values that guide our program are compassion, collaboration, excellence, innovation, and integrity.

Transition

Transitioning from pediatric to adult healthcare can be challenging for patients with congenital heart disease. At URMC's Adult Congenital Heart Disease Program, we understand the importance of a smooth transition process to ensure uninterrupted care and support. Our transition program provides tailored care and assists patients in understanding the complexities of their condition as they transition into adulthood. Our specialists work closely with patients and families to provide the necessary information and resources to make informed decisions about their healthcare. We offer personalized care plans, regular follow-ups, and support to make this process seamless. Our goal is to empower our patients to take charge of their healthcare and achieve the best possible outcomes.
