As pediatric patients reach age 18, there is often a need to shift their medical care from pediatric to adult providers.

  • The transition of care is a process that should begin as early as age 14 in order to prepare patients and families well ahead of time
  • While in the hospital, patients speak with the care team and staff about transition to adult providers.
  • Adolescents have the opportunity to take a guided tour of the adult inpatient floors
  • Information is provided on adult primary care and specialty providers, as well as social work consultation to work through the legal and financial challenges
  • Patients can watch a short educational video to increase their understanding of their personal health and future health care changes
