In our lifestyle management programs, our physicians, team of dietitians, and lifestyle counselors will work with you to help you reach your health goals, whatever they may be. We will provide you with the proven strategies, tools, and support you need to make those meaningful lifestyle changes toward better health.


Weight Loss Program

Lose Weight

24 weeks | 18 one-hour group and 6 individual sessions

Our team of registered dietitians and exercise physiologists will develop a personalized weight loss plan that fits your lifestyle. Covering a variety of nutrition, physical activity, and behavior topics, this program provides tools, support, and encouragement necessary to make and maintain important changes. Maintain Weight Loss

Individual sessions

Work with our specialists on a variety of nutrition, physical activity, and behavior topics specifically for those who have lost weight and want to keep it off or lose more. More about the "Maintain Weight Loss" program



Nutrition Basics

5 weeks | 1-hour group sessions

In our Nutrition Basics program, you'll receive an introduction to the principles of heart-healthy eating and how you can incorporate heart-smart recommendations into your daily routine. Our Nutrition & You program offers a full nutritional assessment and a comprehensive nutritional plan with our registered dietitians. More about the "Nutrition Basics" program

Nutrition & You

Individual sessions

Work one-one-one with a registered dietitian to identify your health and nutrition goals, manage existing concerns and prevent cardiovascular, metabolic and other chronic diseases. More about the "Nutrition & You" program


Lower Cholesterol

Lower Cholesterol

Individual sessions

Work with a registered dietitian to develop a nutrition plan to manage high cholesterol, better understand your cholesterol numbers, and discuss medications, if needed.  More about the "Lower Cholesterol" program


Lower Blood Pressure

Lower Blood Pressure

Individual sessions

Work with a registered dietitian to develop a nutrition plan to manage high blood pressure; receive blood pressure checks at each visit and discuss medications, if needed. More about the "Lower Blood Pressure" program


Fitness programs

Fitness 101: Get Started

5 weeks | 5 one-hour group sessions

Whether you are new to exercise or need a jumpstart to get moving and stay motivated, our exercise specialist builds foundational knowledge about physical activity in a group setting. You will gain a better understanding of cardiovascular fitness, muscle strength, flexibility, and how to prevent injuries. More about the "Fitness 101: Get Started" program

Fitness 201: Get Stronger

6 weeks | 30-minute weekly individual sessions

You will work one-on-one with our exercise specialist or certified personal trainer and receive an initial assessment to evaluate current strength and fitness. The program includes six weeks of 30-minute personalized workouts and recommendations, solutions for setting realistic goals and staying motivated, supervision to avoid injury, and a follow-up assessment. Stop Smoking

Stop Smoking

Individual sessions

Our program is designed to build a customized plan for those that need or want to stop smoking, based on the individual’s specific timetable. The program includes in-person or telehealth counseling, management of withdrawal symptoms, assessment and discussion of medication options if needed, and prevention and management of relapses. Reduce Stress

Reduce Stress

4 weeks | 4 one-hour group sessions

Understand your stressors, make plans of action to reduce these stressors, and identify different pathways to change things that are more difficult to control. More about the "Reduce Stress" program

Mindfulness-Based Stress Reduction

8 weeks | 8 two-hour group sessions and day-long retreat OR 6 weeks | 6 two-hour group session

Learn the benefits of thinking differently about stressful situations, how to live more mindfully with moment-to-moment awareness, and taking time to “be still” through different meditation techniques. More about the "Mindfulness-Based Stress Reduction" program

INDIVIDUAL AND GROUP PROGRAMS are offered throughout the year at a variety of locations, including Clinton Crossings, Corporate Woods, the Medical Center, and the Center for Community Health & Prevention’s main office at 46 Prince Street.
