Social Work Department at Highland Hospital
Part of Your Health Care Team
Our social workers provide services both in the hospital and the community. As a member of the health care team, they are available to you and your family to provide:
Emotional support and counseling.
Help in coordinating plans for your care after you leave the hospital. If you need short-term rehabilitation or a longer stay in a skilled nursing facility, your social worker will help with this process. Your social worker can also arrange an evaluation to determine if nursing care in your home is needed.
Referrals to community resources such as sliding fee-scale aides, family service agencies, geriatric services, transportation services, or community health nurses for home services.
Help with Medicaid, Medicare, and other insurance concerns you may have.
Social workers are assigned to all nursing units in the hospital. Services are available seven days a week from 8 a.m. to 4:30 p.m.
Your physician or nurse can help contact the social worker assigned to you. There is no charge for these services.
