GAMA Serves as Pilot Site of Food Pantry Program

GAMA has been selected to be a pilot site of the UR Medicine Food Pantry program established by a partnership between the University of Rochester Medical Center and Foodlink. The first site was established in Patient Discharge at Strong Memorial Hospital. Recently, the food pantry team created an operational workflow to bring the resource off-site.

Patients identified as experiencing food insecurity or emergency food needs during an appointment will meet with one of the practice social workers, Daniel Fee or Kathy Dodge. Kathy or Daniel will provide the patient with free bags of shelf-stable food to take home. Bags also include a tip sheet for storing food items, recipe brochure, and a SNAP application if needed, along with a QR code to a bilingual satisfaction survey.

The Social Workers will also create an Emergency Food Voucher order and will discuss food stamps and community food cupboard options to help with any ongoing food insecurity needs.

“Our practice will be one of two ambulatory practices to pilot this workflow,” said Corey Hafler, GAMA Practice Manager. “This is an exciting opportunity for us that underscores our commitment to providing equitable, high-quality care to our patients.”
