Social Work/Case Management
Part of Strong Memorial Hospital
A voluntary case management program is available to all patients seen at Strong Memorial Hospital's AIDS Center. We offer services which will provide personalized care planning to all HIV positive patients.
This interdisciplinary team includes:
- A medical provider
- A social worker
- Other staff as necessary
The case management program includes an assessment of medical needs, specifically:
- Medical care in other clinics
- Home nursing care
- Hospice care
- Medical supplies
- Insurance coverage
- Health care proxy planning
and also social needs:
- Housing
- Benefits eligibility and applications
- Transportation
- Substance abuse treatment
- Mental health care
- Insurance coverage
Once the medical and social needs are identified, a case management plan will be developed for you. You and your social worker will monitor the plan to make any continued changes.
