Welcome to the Finger Lakes Alzheimer’s Disease Assistance Center (FLADAC) The Finger Lakes Alzheimer’s Disease Assistance Center (FLADAC) is one of a select few sites that has been designated as a Center of Excellence by New York State for the diagnosis and care of patients with Alzheimer’s disease. The overall goal of the FLADAC is to guide and support individuals with Alzheimer’s disease and their families through the process of assessment, diagnosis and the development and implementation of a treatment plan. The center helps connect patients, families, caregivers and medical providers with local and national organizations that are committed to the diagnosis, treatment, and care and individuals with Alzheimer’s Disease and other types of dementia. The center also offers group and individual training to professionals and provides a clearinghouse of information about resources for Alzheimer’s Disease and other forms of dementia.. The Finger Lakes Alzheimer’s Disease Assistance Center is administered by the University of Rochester. Services are provided for individuals and organizations in the following counties: Chemung, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, and Yates. The FLADAC patient care services are integrated with the URMC Memory Care Program which is located in the Clinton Crossings Medical Center off of Westfall Road. The building is handicap accessible and parking is free of charge.
