There are four steps for enrolling: Completing an introductory survey Complete the consent and baseline forms Enroll in Quit for Life Log on to Quit for Life. Look for these steps as you go through the process. The step highlighted in green is where you are during the enrollment process Can I use a smart phone or tablet to enroll? No. Please use a laptop or desktop computer to enroll. Can I save my answers and finish the survey later? Yes. How do I login to the Quit for Life website? After steps 1, 2, and 3, check your email for a message from firstname.lastname@example.org. That email has your Username and password, and the link you need to log in. I haven’t received the email from email@example.com. It may take up to 24 hours for the email to arrive. Please check again in a few hours. Also check your junk folder or SPAM filter. If you do not receive the email after 24 hours, contact us for help. I received the email from firstname.lastname@example.org but I cannot find it. Please contact the Study Coordinator at 585-276-6243 or WATI@URMC.Rochester.edu How often do I have to use Quit for Life to be in the study? You must log on to Quit for Life at least once in order to fully enroll in the study. How much you visit Quit for Life after that is up to you. Do I have quit smoking to stay in the study? No, you may stay in the study whether you quit smoking or not. To stay in the study you need to complete the consent form, four surveys, and log on to Quit for Life at least once. What if I don’t want to answer all the questions on the surveys? You are free to decline to answer any questions you do not wish to answer or to stop participation at any time. To enroll in this study, you must be: at least 18 years old; smoking at least 1 cigarette per day on at least 5 days per week or at least 5 cigarettes on 1 day per week; planning to stop smoking on your own within the next 3 months or have quit smoking in the last 7 days . enrolled in a community college. Can I join the study if I’m thinking of quitting in the future - like maybe in the next 6 months? No. This study is for smokers who are ready to quit right now, or in the next 3 months. Feel free to come back later to join. Why didn’t I get into the study? You will be excluded from the study if you are: Less than 18 years old. Not enrolled in a Community College Not a smoker Smoke less than 5 days a week and smoke less than 5 cigarettes on the days that you smoke Are not ready to quit within the next 3 months Do I have quit smoking to stay in the study? No, you may stay in the study whether you quit smoking or not. To stay in the study you need to complete the consent form, four surveys, and log on to Quit for Life at least once. What if I don’t want to answer all the questions on the surveys? You are free to decline to answer any questions you do not wish to answer or to stop participation at any time. Can I enroll in the study more than once? If you tried to enroll in the study and did not qualify, you can try again when you think you do qualify. For example, if you tried when you were too young, you can try again when you turn 18. If you’ve already participated in the study by completing all four steps, you cannot participate again. Who is sponsoring this study? The University of Rochester is receiving funds from the National Institute of Health (National Cancer Institute) to conduct this study. Will I get paid for being in the study? As a “Thank You” for your time and effort, whether or not you have quit smoking, you will receive: A $10 gift card for completing the 1st online follow-up survey 1 month after you enroll A $15 gift card for completing the 2nd online follow-up survey 6 months after you enroll A $20 for completing the 3rd and final online follow-up survey 12 months after you enroll, for a possible total of $45. If you are selected for biochemical sample collection (breath and saliva), your gift card will be processed after the collection is complete. Can I get paid in cash? You can be paid with a check or a Target gift card.
