SCDDs Person-Centered Planning in the Foster Care System project has developed a variety of resources including a Future Planning Workbook, Glossary of Disability Terminology, Person-Centered Planning Tip Sheets, and more.

The project, funded by the NYS Developmental Disabilities Planning Council, has aimed to develop a long-term, sustainable plan for the use of person-centered planning in the foster care system and thereby increase quality of life outcomes for youth with intellectual and/or developmental disabilities (IDD) served in the foster care system. Five NYS counties, including Westchester, Orange, Rockland, Monroe, and Cattaraugus, in addition to the New York City Administration for Children’s Services, have participated in the project’s development.

The project team is in the process of finalizing online person-centered planning training videos and working to make critical resources available in Spanish. Collaboration to ensure distribution of developed resources continues to ensure sustainability of the project across New York State.
