Community Home Care Services

Our concern for your health care continues after your discharge. We understand that returning home or to another familiar environment often provides an immediate source of comfort and healing and we want you to benefit from this. You may be ready to leave Strong Memorial Hospital but still need additional health care services such as home health care or assisted living to ensure your safety. Throughout your hospital stay, your potential needs for these services will be assessed by your primary health care team and all of your questions and concerns will be addressed quickly and efficiently by the home care staff. At the time of discharge, your specific discharge needs will be assessed and all information will be shared with you and your primary support person(s).

Home Care Liaison Nurses and Home Care Coordinators

If you need home care services, our Home Care Liaison Nurses and Home Care Coordinators will speak with you and your health care team to coordinate a safe, cohesive discharge plan. Whether you have a local residence or live outside New York State, we will be able to offer you choices of available nursing agencies.

If you live within Monroe County or in Livingston County the four agencies listed below are available to help you meet your home care needs. Our nurses also assist with nursing home placement by preparing the assessment tool called the Patient Review Instrument or PRI. This instrument is required to determine what services you may need and which nursing home could offer the best and most comforting fit for your personal situation out of the available options.

For home care needs within Monroe County you may choose:

  • UR Medicine Home Care: You may need the assistance of a nurse, physical therapist or nurse’s aide once you arrive home. UR Medicine Home Care, an affiliate of the University of Rochester Medical Center, provides quality home care for newborns to seniors, whether acutely ill, temporarily disabled or requiring long-term care. Many services are covered by insurance, including Medicaid, Medicare and private insurance. Call UR Medicine Home Care at (585) 787-2233 or ask your nurse for more information.
  • Lifetime Care
  • Home Care of Rochester (HCR)

Home Care Liaison nurses or Home Care Coordinators are on site every day from 8:00 a.m. to 4:30 p.m. and are able to arrange for things you will need at home. They are easily contacted via phone and pager by the nursing staff on all units in the hospital. Simply ask your nurse to contact a Home Care Liaison nurse or Home Care Coordinator to meet with you. They will arrange for:

Visits by:

  • Nursing
  • Physical, Occupational and Speech Therapy
  • Social Worker
  • Home Health Aides

Services:

  • IV Infusion
  • Tube Feeding (all formulas)
  • Wound Care
  • Medication Management
  • Symptom Management
  • Lab draws
  • Evaluation and Assessment throughout your recovery
  • Oxygen: for travel and in the home

If you plan to receive home care services, the information you discuss with the nurse will be listed on your After Visit Summary or AVS. Your AVS will include the agency that you will be going to along with the agency’s telephone number and services provided. You can also refer to the AVS or contact the agency directly to confirm your first scheduled visit. You may also view your entire AVS in your MyChart.

Assisted Living Services

Home care is one type of long-term care that helps you or a loved one stay home successfully and safely. Another long-term care option is an assisted living facility. From enriched living to a life care community or nursing home, there are a range of environments available to meet your health care needs and maintain your well-being.
