Forms are used throughout our site to request information, contact departments, sign up for newsletters, etc. To request a form, please submit a ticket.
Event registration is handled by the Center for Experiential Learning.
We will work with you to determine your requirements and build you a form that meets your needs.
All forms deployed to public-facing website require review.
We work closely with the URMC privacy and security office and ensure that all forms are secure and comply with all guidelines.
By default, CAPTCHAs are not added to our forms as they can be considered user hostile. If you observe an increase in SPAM being submitted via your form please contact us and we will install a CAPTCHA on your form.
