About Us

The Center for Community Health's programs and services are designed to help people live healthier lives. They include the following:

Cancer Services Program of Monroe County

The Cancer Services Program of Monroe County, previously known as the Health Partnership, pays for breast, cervical and colorectal cancer screenings for uninsured and under-insured adults, ages 40 to 64. Screenings are provided at over 100 provider partners’ offices throughout Monroe County. Women 19-26 can also receive HPV vaccine at several provider offices.

Emerging and Healthcare Associated Infections: Surveillance and Prevention

The Emerging Infections Program  (EIP) is part of a national effort to provide population-based communicable disease data for surveillance of disease patterns, evaluation of vaccine programs,  and to identify populations at risk for severe infection and those in need of screening and preventive care.  EIP projects include studies of risk factors for influenza disease requiring hospital admission, methicillin-resistant Staphylococcus aureus  (MRSA), and Clostridium difficile in healthcare and community settings.  The Healthcare Associated Infections Collaborative develops quality assessment methods to monitor central line-associated blood stream infections outside the ICU and related projects.  Work in the Center for Community Health’s EIP-HAI group serves as a foundation for research studies and informs public health practice and policy at the local, state and national levels. Health Research

In addition to the Center's programs, we actively support the URMC's research efforts. As the community engagement arm of the Clinical and Translational Science Institute, we help ensure that the community's persepective is incorporated into the design of research studies and the way they are conducted. The community is key to our success, and we are collaborating with community partners to:

  • Increase community input to the identification of research priorities and to the way research trials and studies are designed, promoted and implemented.
  • Help investigators and their research colleagues better understand and incorporate community persepcitves into their research.
  • Increase the participation of primary care practices in research to improve patient care and outcomes.
  • Increase the level and diveristy of volunteer participation in research.

Healthy Living Library

The Center’s Healthy Living Library is open to the public and offers a quiet, comfortable place for reading and research on health topics.  In addition to books, DVD’s, pamphlets, and anatomical models and charts, the Healthy Living Library features computer workstations with simple, targeted access to high-quality, trusted health information on the web.  The Healthy Living Library and the Center’s conference room are also available at no charge – to our community partners for meeting space.  Bright and attractive, these rooms are ideal for meetings or small gatherings.  Internet access and conference-call connections are available.

Healthy Hero Outreach Project

The Healthy Hero Outreach Project is a family engagement initiative, which targets ethnically diverse families with children ages 2-10 years old living in the City of Rochester, Greece, Irondequoit , and Gates.  A team of outreach workers provides interactive presentations on the Greater Rochester Health Foundation’s 5-2-1-0  Be a Healthy Hero campaign, which help parents and families build healthy eating and active play into their everyday routines.

Rochester Walks!

The Center and Action for a Better Community have partnered to implement “Rochester Walks!,” a grant-funded program to promote walking and physical activity in selected city neighborhoods. The grant is administered by the City of Rochester with funds from the New York State Department of Health.

Healthi Kids

The Healthi Kids initiative, led by the Finger Lakes Health Systems Agency and supported by the Center for Community Health, Department of Pediatrics, and the Children’s Agenda, is a community-based coalition advocating for five policy and practice changes: better school food; safer, more accessible play areas; healthier food standards in early child care settings; increased in-school physical activity; and policies that support breastfeeding.

Healthy Living Center

Helping people improve their health by making meaningful and lasting changes in their lives is the focus of our new Healthy Living Center. Through individual counseling and/or group support, we help individuals prevent and manage diabetes, enhance physical activity, improve diet and nutrition, eliminate exposure to tobacco, increase medication adherence, and reduce stress.  Our physicians, nurses, dietitians, and counselors use proven strategies to help clients start and stay on track to improve their health.  Our goal is to help individuals discover their own personal desire to make healthy changes in their lives. Healthy Living Program/Vida en Salud

The Healthy Living Program and its Latino counterpart (Vida en Salud) are community-based fitness and health education programs aimed at helping African Americans and Latinos adopt healthy lifestyles.  The programs emphasize physical activity and healthy eating.  They are run in partnership with the YMCA in local churches and community organizations.
