Overview

This clinic meets the needs of a local frail aging population, especially those with significant comorbid disease, functional decline, or requiring higher levels of personal care necessitating transition to alternative living settings. The clinic meets goals of comprehensive assessment as outlined by the National Institutes of Health: improved diagnostic accuracy, guidance in selection of interventions to restore or preserve health, and aid in choosing an optimal environment for care.  

Patient & Visitor Information

Patients must be 65 or older, have a primary care physician, and are not already seeing a geriatrician. Patients must be approved following the completion of a clinical preview procedure by both our Admitting Coordinator and/or Clinical Admissions Coordinator. Referrals are evaluated based on clinical requirements, psychological needs, and the quality of treatment. To begin the intake process, please contact (585) 760-6353.

First patient appointments last around 1.5 hours. Patients are asked to return 1-2 weeks later for a one-hour closing appointment.

Services

A comprehensive assessment is performed by an interdisciplinary team consisting of geriatricians, nurses, neuropsychologists, pharmacists and social workers. Personalized recommendations are provided to patients, families, caregivers, and primary care physicians as a result of the consultative office visit, with referrals made to a variety of community resources and service agencies in the Greater Rochester area.

Method of Payment Accepted: Most Insurances and Medicare Documentation 

Documentation Required for Service: Must have an established primary care physician and a record request will be sent to that physician's office with patient consent prior to appointment for the team to review. 

Instructions for Referrals: Our primary referral base is the Rochester area and Monroe County, but referrals are accepted from across the region. Call (585) 760-6353  to start intake process. Clinic coordinator will return your call to complete intake assessment over the phone. 

Waitlist/Availability: About 3 months, first priority given to Monroe County residents but will accept referrals from surrounding counties.
