New Patient Information and Forms

Welcome to the Allergy, Immunology and Rheumatology Practice at the University of Rochester. First appointments are always somewhat stressful. You might ask yourself, "Will I get lost and be late? If I do get lost, will I then have enough time to fill out all the paper work? Will I have enough time to discuss all my concerns with my new doctor?"

In order to make the most of your time with us, we are providing you with the information you will need: A map, directions, and the forms you can fill out ahead of time.
