Services & Programs

  • ​Complex Care Center – Adults with complex childhood onset conditions can receive primary medical care and specialty services in one location.
  • Sickle Cell Program – For Providers: "What Health Care Providers Need to Know About Caring for Adult Patients with Sickle Cell Disease"
  • Adult Cystic Fibrosis Center – Accredited by the Cystic Fibrosis Foundation (CFF), we are a premiere Cystic Fibrosis Center serving Upstate New York.
  • Care Management/Health Home & Care Management – A Health Home is a care management service model whereby an individual's caregivers communicate with one another so that the patient's needs are addressed in a comprehensive manner.
  • Applied Behavior Analysis (ABA)/Intensive Behavior Support – A therapy based on the science of learning and behavior.
  • Dental Services – Dental services are available at the Complex Care Center. Our goal is to improve patient care for a population with low access and significant medical challenges. Patients do not have to be receiving medical care at the CCC in order to come for a dental appointment.
  • Nutrition Services – We can provide information and general nutrition support, management of tube feeding/formulas, TPN, consultation services.
  • Adult Inpatient Services – While the goal of our division is to help patients thrive in the community, when hospitalization is required, we are able to provide care in the inpatient setting.
