Telehealth

The Center for Community Health & Prevention offers a convenient new way to access some of our individual services using telehealth, a secure web-based video conferencing tool. Telehealth provides an innovative and efficient way to attend appointments with our providers, from the privacy and comfort of your home, office or any remote location.

To ensure your security, we use the HIPAA-compliant Zoom application for telehealth sessions. This easy-to-use technology is available for computers and feature-rich mobile apps on iOS, Chrome OS and Android devices.

Before Your Telehealth Appointment

  • Schedule. Call us at (585) 602-0720 to discuss your scheduling options. Once you select your telehealth appointment date and time, our office staff will send you an email with your appointment details and login instructions. 
  • Decide which device you will use. Zoom may be used on PC, Mac, iOS, Chrome OS, or Android devices. The device you choose will need access to the internet, your email service, and a webcam/microphone.
  • Choose a private, well-lit location. You will be required to use both video and audio, so be sure to select a quiet location where you can comfortably hear your provider and speak without distraction.
  • Make sure you have a strong internet connection. We recommend using WiFi, as cellular networks (3G, 4G, LTE, etc.) will utilize a considerable amount of data.

For Your Telehealth Appointment

To use a PC or Mac computer with webcam and microphone:

  • Open the email containing your appointment information.
  • Click the link in the message. The Zoom program will begin downloading automatically.
  • When prompted, enter the session password provided in the email. This password is always the 8-digit date of your scheduled session in MMDDYYYY format.

To use a mobile device (iPhone, Chrome or Android device, iPad or tablet, etc.):

  • Download the free Zoom Cloud Meetings app from iTunes (iOS), Chrome Web Store, or Google Play (Android).
  • Open the email containing your appointment information.
  • Tap the link in the message. The Zoom app will open.
  • When prompted, enter the session password provided in the email. This password is always the 8-digit date of your scheduled session in MMDDYYYY format.
