Our clinic-based Blood Pressure Advocate
Program aims to help adults diagnosed with high blood pressure get their blood pressure under control by making positive and permanent changes in their behavior and lifestyle. Our program is part of the community-wide High Blood Pressure Collaborative.
Our trained advocates are embedded in local clinics and serve as the link between the health care providers and the patients.

Our trained advocates work one-on-one with patients, most of whom are referred by patients’ health care providers. Patients can also request to meet with an advocate on their own. The services are FREE for patients.
Advocates provide the following support:Summer series of community health talks (Grand Rounds), scheduled June 7 through July 26, 2013.
