The Memory Care Program provides diagnosis, treatment, and care of neurodegenerative conditions that affect the elderly and the aging nervous system.
Our program specializes in treating older patients with:
- Memory loss
- Dementia
- Cognitive dysfunction
- Alzheimer’s disease
The Memory Care Program provides
- Assessment, evaluation, and diagnostic workup
- Ongoing medication management and help with challenging behaviors
- Recommendations for treatment
- Information, education, and support for their care partners
- Referrals for community resources
- Opportunities to participate in research studies
What to Expect at Your First Appointment
Initial visits typically last at least 90 minutes and are divided into three parts.
Part 1: Intake and Cognitive Assessment
The patient will be seen initially by our patient care technician who will:
- Record vital signs
- Review and reconcile your current medications
Conduct cognitive screening conducted utilizing a variety of measures.
Please bring a current medication list to your appointment.
During this time, the person accompanying you may meet separately with the physician to provide additional history or discuss concerns.

Part 2: Physician Evaluation
The patient will then meet with the physician to:
- Review your symptoms and medical history
- Complete a physical examination
Part 3: Review and Next Steps
The patient and their companion will discuss the results of the evaluation with the physician and recommended next steps.
