I found the MD very personable. We discussed options for treatment. Plus what type of activities should be avoided. We discussed pros and cons of various treatments.

The Provider did not introduce himself. It is such a basic, fundamental aspect of doctor-patient relationships - it amazes me that there are still physicians that do not understand this and don't do that. Please pass that on to him. Thank you.
