Strong Ties Community Support Program

Mission of Service

Strong Ties is an outpatient clinic that provides comprehensive treatment for adults with mental disorders and related comorbidities. Strong Ties provides psychiatric support and clinical treatment for adult patients and their families who need ongoing outpatient treatment and community support to reduce impairments, prevent relapse, achieve treatment goals, and keep the patient safe in the community.

Target Population

Adults, ages 18 - 65, with serious mental disorders.

Description of Service Providers

Our multidisciplinary healthcare team is comprised of psychiatrists, nurse practitioners, licensed and registered nurses, allied with social workers, primary therapist, and care advocates.

Types of Services Provided

Strong Ties Outpatient Clinical Services include:

  • medication management clinic
  • supportive, individual and group therapies

Assertive Community Treatment (ACT)/Forensic Assertive Community Treatment (FACT): A specialty program multidisciplinary team delivering mobile treatment, providing intensive outreach care for the communities most impaired and challenged individuals in the Monroe County region. The FACT team focuses on serving those justice-involved individuals with serious mental disorders by integration with the court system.

Project Link: This specialty program is designed to prevent arrest and incarceration among multicultural populations with mental illness and who are linked to criminal justice system. Services include medication management, case advocacy and supportive group therapies.
