  • Initial social work assessments to identify family strengths, perceived problems, available resources, and areas that require specific project focus
  • Regular home visitation by paraprofessional outreach staff who focus on accessing resources, preparing mothers for child birth and parenting, enhancing daily life management skills, educating mothers on how to live healthy lives, and promoting parent/child attachment, child safety, and child development
  • Development of individual and family service plans during pregnancy and the first six months of your baby's life
  • Enrollment of the family, as needed, in health care, mental health and/or substance abuse treatment services
  • Transportation to medical and social service appointments
  • Social work intervention in situations requiring professional expertise (i.e. suicide threat, domestic violence and child abuse)
  • Provision of parent education and support groups
  • Referral and enrollment of family members in other community programs as needed
