Baby Love

The Baby Love program is a social worker/community health worker, home visitor model that assists pregnant women and teens achieve healthy births and improved health outcomes. The program seeks to partner with the family to address barriers to care and social determinates of health. It is open to pregnant patients living in Monroe County, who are part of a URMC affiliated practice.

Services

• Assistance in keeping prenatal and other health care appointments
• Help in preparing for the birth of the baby
• Supportive home visits with focus on the Partners for a Healthy Baby curriculum
• Linkage to behavioral health and other needed services
